Senior Citizen Certificate

Application form for Senior Citizen Certificate is available at DC’s Office. Only citizens attaining the age of 60 can apply. Form can also be downloaded here Senior Citizen Certificate Application Form .

Attested copies of the following documents are to be submitted along with the application form duly filled in:-

• Two Passport Size Photographs
• Birth Certificate / Age Certificate from Government Doctor
• Pension Payment Order (for retired government servants)
• Any one of Residential Proof (EPIC/ Ration Card / patta / Electricity Bill / Telephone Bills) Blood Group
