How to Apply for

  1. Apply for Scheduled Tribe/ Caste Certificate?
    Application form for Scheduled Tribe/ Scheduled Caste Certificate is available at the DC's Office. Form can also be downloaded from the district web site.
    Attested copies of the following documents are to be submitted along with the application form duly filled in:-
    • Two Passport Size Photographs
    • Birth Certificate/ S.S.L.C. Admit Card
    • Any one of Residential Proof (EPIC/ Ration Card/patta/Electricity Bill/Telephone Bills)
    • Parent's S.C./S.T. Certificate
    • If adopted, Adoption Documents (Court Order/ Registered Adoption Deed)

  2. Apply for Permanent Residential Certificate?
    Application form for Permanent Residential Certificate is available at the DC's Office. Form can also be downloaded from the district web site.
    Attested copies of the following documents are to be submitted along with the application form duly filled in:-
    • Two Passport Size Photographs
    • Birth Certificate/School Certificate
    • Any one of Residential Proof ( EPIC /Ration Card/patta /Electricity Bill / Telephone Bills)
    • Certificates of all institutions in Meghalaya where Studied in
    • Supporting Documents why Permanent Residential Certificate is required

  3. Apply for Recruitment in Armed /Para Military/ PoliceForces?
    Application form for Armed Para Military Certificate is available at DC's Office. Form can also be downloaded from the district web site.
    Attested copies of the following documents are to be submitted along with the application form duly filled in:-
    • Two Passport Size Photographs
    • Birth Certificate/School Certificate
    • Any one of Residential Proof (EPIC/Ration Card / patta/ Electricity Bill / Telephone Bills )
    • Certificates of all institutions in Meghalaya where Studied in
    • Proof of migration (if applicable)
    • Supporting Documents why Armed/Para Military Certificate is required

  4. Apply for Senior Citizen Certificate?
    Application form for Senior Citizen Certificate is available at DC's Office. Only citizens attaining the age of 60 can apply. Form can also be downloaded from the district website.
    Attested copies of the following documents are to be submitted along with the application form duly filled in:-
    • Two Passport Size Photographs
    • Birth Certificate / Age Certificate from Government Doctor
    • Pension Payment Order (for retired government servants)
    • Any one of Residential Proof (EPIC/ Ration Card / patta / Electricity Bill / Telephone Bills)
    • Blood Group

  5. Apply for Income Certificate?
    Application form for Income Certificate is available at DC's Office. Form can also be downloaded from the district website. Attested copies of the following documents are to be submitted along with the application form duly filled in:-
    • Pension Payment Order/ Certificate from Treasury Officer (for retired government servants)
    • Proof of Salary in form of Affidavit (for non-Government applicants)
    • Any one of Residential Proof (EPIC/Ration Card / patta / Certificate from Area Development Council / Electricity Bill/ Telephone Bills)
    • Salary Certificate from Employer in case parent(s)/ Applicant are employed
    • Proof of requirement for the Certificate, if applicable
    • Declaration if either of them is not employed

  6. Application of the Association/Club for Registration under the Societies registration Act XII of 1983
    Guidelines for Submission of Memmorandum and the Rules and Regulations on the Association for Registration under the Societies Registration Act of XII 1983.Two set of documents are required tobe submitted to the Joint Reigstrar of Societies for Registration of the Association/Club.Memorandum:- (Section 4) The Memorandum of Association should provide clearly the following:-
    • Name of the Association, Address and Post Office, Elaka also indicate Syiemship, Sardarship, etc.
    • Date of Establishment of the Association
    • Aims and Objects
    • The Names, Address and description of the Executive/ Governing Body
    • Signature of not less than 7(seven) of the Members of the Executive Committee/Governing Body on the body of the Memorandum, Rules and Regulations ( Constitution)
    • Procedure for holding the General Meeting and its power and functions
    • Procedure for conducting General Meeting/Executive Committee its power and functions
    • Procedure for expulsion of the Members of the Association/ Club
    • (Section 15) The appointment of a qualified Auditor by the General Meeting for Audit of the Accounts of the Association
    • (Section 19) The designation of the Office Bearers of the Association who sue or be sued on behalf of the Association (Section 6) of the Societies Registration Act 1983
    • (Section 27) The Disposal fo the Assets of the Association on the winding up of the Affairs of the Association
    • Amount of Subscription/Admission fee of the Members
    • Procedure of Election of the Executive Committee and their terms, the Election of Office and also their re-election
    • Name of the Bank/Post Office where the fund of the Association is deposited
    • Amendment
    • Any other provisions which appears to be necessary for the management of the Association should be provided in the Rules and regulation

    Please Note also:-

    1. Correction made in the Rules and Regulations and in the Memorandum of the Association should be initiated by the Secretary/Chairman/President of the Office Bearers duly authorized
    2. The Rules and Regulations,Constituion of the Association should be be typed neatly and on thick papers for records
    3. One copy of the Treasury Challan showing the deposit fee Rs. 250/- should be deposited. The Memorandum and the Rules and Regulations along with Treasury Challan should be submiited to the Joint Registrar of Societies
    4. The Official Seal (Round Seal) of the Association on every page of the Rulesa and Regulations should be affixed
    5. The Rules and Regulations,Constitution of the Association should be signed in every page by at least 3(three) Members of the Executive Committee or Governing Body
    6. N.O.C. from the Headman.
